The Cavern of the Incarnate can be found if you travel to the city of Vos, and then walk directly west until you get to some mountains. When you reach some mountains, levitate one mountain range, then follow a path south thats runs between two mountain ranges. When you reach the end of the path there will be a bit of a clearing.

If you walk around the edge of the clearing you should be able to find the Cavern of the Incarnate. By the way, I think it has to be either 6pm or am for you to be able to enter the cavern.
