Hi I have a question :

How do you get vincents last limit break and his ultimate weapon?

Please answer this because I've ben stuck on this for ages!

When you are on disc 2 visit Lucrecia's cave. To get the the cave you can either use the submarine or a chocobo with mountain crossing abilities. The lake which the cave is by is just East of Nibelheim. To get there with the submarine look for a narrow passage underwater which leads you to a small underwater cove with nowhere to go bu back the way you came. Take the submarine above water here. Dock the submarine and walk into the cave, you can't miss it. When entering the cave make sure that you have Vincent in your party. When you enter a woman called Lucrecia is in there. Then you enter a cut-scene which explains a lot about Vincent's life and why he was locked up in a coffin in the Shinra Mansion. After the story exit the cave. Come back here on disc 3 again with Vincent in your party. Where Lucrecia was standing before will be Vincent's ultimate weapon the Death Penalty and his level four limit break Chaos.

When you have the submarine get as close to the gold saucer desert as you can then dive under.There should be a small passage in front of you.Go through and go up at the end of the tunnel. You can dock and there is a waterfall go inside.You must have vincent in your party.There will be a scene.When its done leave do something because after an hour or two go back and you will find chaos his ultimate limit and death penalty his best weapon.You can also reach the waterfall with a mountain chocobo.

Visit Lucrecia's cave on disc 2 with Vincent in your party. You can either use a chocobo with mountain crossing abilities or you can use the submarine. The lake is East of Nibelheim. To get there with the submarine simply look for a cave underwater and go through it until you reach a circular area where you can raise the submarine to the top. When you have done so dock it and enter the cave(you can't miss it). Vincent steps foreward and talks to Lucrecia. Then it shows a cutscene of how Vincent ended up in a coffin the the Shinra Mansion. After this exit the cave. Come back on disc 3. Vincents ultimate weapon Death Penalty and his level 4 limit break Chaos will be there.

You need to go under a waterfall (by going into a cave underwater with your submarine on disk 2). Be sure to have Vincent in your party. Once you are behind the waterfall, there will be a short scene about Vincent's past. Come back here on disk 3, and check the the stage. You will recieve his ultimate weapon and his final limit break. Hint: I heard somewhere that you go behind the cave on disk 2 and watch the scene. After that, you go out and fight ten battles. After the tenth battle, go back inside the cave to get Vincent's ultimate weapon and his final limit break (all on disk 2!) I tried this, and it worked. The only problem with doing it this way is that it somehow glitched my game, and made it to where Barret and Vincent to about 100 damage each time they attack, when they were supposed to do around 4,000. I don't recommend getting Vincent's weapon and limit break this way, so do it the normal way (stated above).

All of the other answers do say that you have to go to the cave on disk 2, then again on disk 3. But, instead of waiting until disk 3, you could just go back after defeating Diamond weapon(ie. The one that comes out of the sea and walks towards Midgar).
