Ok, there are two heart peaces there that you could be talking about so i'll tell you how to get them both=)
1. Enter Death mountain from the top-most entrance next to the great fariy's cave and go forward until the cliff ends, climb down to the alcove to find a heart piece.
2. As a kid you need to plant a magic bean next to the fire temple enterance then come back as an adult and get on the plant and ride it until you come to a heart piece, get off and get the heart.

You need to plant Magic Beans as a Kid near the entrance to Dodongo's Cavern and as an Adult, ride the Flower thing up and jump off of it to get that Heart Piece.

Hi There!
There are two heart pieces on ledges you might be asking about:
A) One Above Dodongo's Cavern
B) One way up inside Death Mountain
Answers
A) Plant a magic bean as a kid outside Dodongo's Cavern (remember you can get a golden sculta from soft soil by releasing bugs nearby)
B) Learn the Bolero of Fire as an Adult then warp back as a kid and plant magic beans (again, remember the golden sculta)!!!

1) The one on top of the mountain thing you need to play the song that takes you to the center of the crator as a kid. Plant a magic bean in the soft soil. (note make sure you get the gold skullita out of it first. Release bugs next to it.)
2) As Adult or Kid go to the highest point of the crater and walk slowly towards the edge. It might take a few tries.

Yeah, like the other guys says you have to ride the bean flower to get there.
But there IS another way.
Go to the mountain platform where you found the bomb flower as a kid and do a backflip over the lowest part of the fence (Yeah, I know it's a bit tricky to land correct) and then grab the piece of heart.
