You have to either complete driving school with all gold medals, or turn on the all traffic is rare cars cheat.

You can get all gold in Driving School and you will unlock the HotKnife.Another way is to use the cheat Clown Mode too. The cheat will make CJ look funny, and there will be different cars when you type the cheat in.The cheat for clown Mode is:( Y,Y,L trigger,X,X,B,X,down,B)

The two quickest and best ways to get the hotknife are:

Get all golds in the driving school and it will be parkd outside.

Or, use the fast/rare cars appear regularly cheat, and roam around, you will find one eventually, although they are still pretty rare when you use this cheat, it depends where you go.
