Yes, like the others have said, you actually have to participate in Sim Dragon mode and beat it to receive the item 18's kiss. I know, I know, it really sucks because Sim Dragon is the worst mode mode on the game, and can be really hard in some parts. My advice to you would be to pick a strong character, one such as Gogeta Super Sayian 4, or Hirudegarn. You can pick whatever character you like, but the stronger the character is, the less trouble you will have with your opponents. Be sure to explore for Z items to give to your character.
