The reason for that is really simple - the version of your neighbor's town you are playing is not really their town, it is a save of their town that is stored on the EA server.

When you visit your neighbor's town and the save is over a day old, the Sims in it are timed forward for all their needs to the time on the clock when you actually visit, which means none of their needs are fulfilled and they are stinky, tired, and hungry.

I hope that clears it up for you?

With the baby, when you change the diaper, it only fills the hygiene meter, not the toilet one. If you want to fill both, then wait until the baby's toilet meter is red then it will wet itself, afterwards which the toilet meter is filled. Then you change his/her diaper and both meter are filled

CMBF answered:

Because the game you are playing when you visit them is an old save that was placed in the cloud - so when you arrive the game applies the time differences to the world that would apply between the time stamp for the save, and now.

So all the needs expire and you get what you get.
