Without doubt the best starting keepsake in Elden Ring is the Golden Seed which is an item you can use at a Site of Grace to increase the number of uses for your Sacred Flasks. This basically allows you to heal or recover FP more often. You may also want to consider the Stonesword Key which will enable you to access a new area. It is a one time use item that breaks when used, you can purchase more Stonesword Keys from Patches in Murkwater Cave for 5,000 Runes each.
Note: If you decide to pick the Stonesword Key it is recommended that you do not use it on the imp seal lock in the starting area as this lock hides a very high-level area which your character will not survive long in.
