Byrgenwerth is a required story location in the game. To access Byrgenwerth, go to the arena where you battled the Shadow of Yharnam and continue on passed the map lantern.

After passing the lantern kill the Giant Fly Man and then turn left. Head down and go up the stairs at the end then go down again here as it will show you a lakeside view of the area. Pick up the Madman's Knowledge from a corpse nearby. There will also be a Giant Centipede that will shoot at you so beware. After that, take the Great Orb’s Wisdom from another corpse nearby.

From here you will find a building by the lake. You may enter the building by going in at the side.

Upon entering you will find the library building, there will be a chest that will give you Pearl Slug. There will also be a hunter dressed in a Toga so beware. Defeating the Hunter will give you Blood Dreg. At the end the room, there will also be a corpse that will give you a Sedative. At this point you may also open the door that was previously locked at the edge of the room.

Climb the stairs to the second floor and there you will find a chest that will give you a Student Uniform Set. There will also be a locked door in this area.

By the bookshelf, you will read a passage.

Climb the ladder and take the Lunarium Key. Climb the stairs to the top and pick up an Empty Phantasm Shell from the chest.

With the key you may open the door on the 2nd floor where you will find Master Willhelm sitting on a rocking chair pointing towards the edge of the platform.

Take a plunge into the lake and you will enter the Moonside Lake where you will battle Rom, the Vacuous Spider, who isn’t aggressive at all.

The minute you hit Rom, it will immediately summon a dozen of spiders around it which will protect and surround Rom--these spiders won't attack you deliberately but will try their best to block you from getting near Rom. Once you get to hit Rom a couple of times, it may disappear and spawn far in the lake and will cast bolts of ice at you. It is important to always make quick dodges here. Rom may also roll over and summon ice bolts from the water.

Rom is weakest from the side so move fast and hit him here. Once you are successful in defeating Rom, you will be rewarded with Kin Coldblood after.

After the battle, proceed to the end where you may find a woman in a bloody dress facing the red moon.

A cut scene will commence showing the engorged moon and a ritual secret will be revealed.

You will be transported back to cathedral ward the entrance to lecture building.

After battling Rom and the appearance of the Blood Moon, you will notice that some of the NPCs in their home (with red lanterns) will no longer be around. In fact, if you visit Gilbert from central Yharnam, you will find that Gilbert has already transformed into a beast.

You may even visit the little girl who gave you the music box before and she will talk about her missing sister and when she asks you to help her find her sister. After exploring the Aqueducts in central Yharnam you may give the Red Bloodied Ribbon to the little girl in the window. After that, the little girl will sob after learning about her sister’s demise. Later on, you will hear the little girl giggling, as if turning mad, about the ribbon you just gave her.

After the battle with Rom, you may also revisit iosefka at her clinic. If you didn’t battle her earlier, you will find Iosefka on an operating table. You may kill Iosefka now and in doing so, you will receive a Third Umbilical Cord.
