If I get spotted then killed and I choose exit not continue does that count as an alert in the end of the game when I retry?

You must exit the game. Do not continue as this will count as an alert. If you get spotted I suggest just pressing all of the shoulder buttons (L2 R2 L1 R1) and select and start at the same time. This will reset the game and allow you to reload the latest save game.
