Chapter 4: An Endless Battle

After the scene, you'll be partnering up with Igraine. Go downstairs and follow the straightforward path until you reach a blocked gate. Examine it then backtrack to the previously closed gate. Kill the two guards there continue inside. The path is straightforward and you'll encounter a handful of rebels along the way. Continue until you reach the generator room. There are more enemies inside to be careful and aim carefully. Once cleared, continue ahead until you reach the gate leading to the London Hospital. Open the gate to trigger another scene.

After the scene, you'll be inside of the hospital. Leave the lobby and enter the next corridor. A couple of rebels will appear so take them both out. Once done, interact with Igraine and you'll split. Head downstairs to investigate the basement. While in the basement, search around until you trigger a scene with a lycan. While you're running away from it, keep shooting until it corners you. Complete the simple QTE to drink your Blackwater and restore a bit of health. Make your way toward your rifle past the bookshelf. After reaching it, another scene and QTE will follow. This will repel the powerful lycan for the meantime.

Find your way out of the room then continue upstairs. Once you've reached another gate, open it to trigger another QTE. Walk towards Igraine and try to remove the rubble to trigger a fight against the Elder Lycan. For the first round, you have to time your attacks and use the right analog stick to evade the Elder's attacks. Keep attacking and evading until you deal enough damage and force it to enter the second phase of the battle. Use your Blacklight skill when prompted and press the QTE buttons correctly as they appear on the screen. The battle will drag on with additional scenes and more QTE prompts. Just follow them and you'll defeat the Elder.

Against a menace.

After the scene, examine the hospital room for clues. Head to the end of the room and pick the lock on the red door. Inside, examine the hanged uniforms then open the chest for another Phonograph (The Anarchists 29.09.86) Once done, head back to Ingraine and assist her out of the hospital.
