Provided the Scientist is out of the way, open the door to the Dotted Hole by using Cut. Fall through the hole, then on the floor below use the hole to the north, then west below, then east, then south. At the bottom is the Sapphire, but the Scientist will snatch it.

Get into the Dotted Hole by using cut. The saphire is in the dotted hole. Once you are in the Dotted Hole fall down the hole. Then fall the into the top. Then left, then right, then bottom. You will find the saphire. The altering cave is a place where you can find rare Pokemon if you have this thing. I forgot what it was.
