Once you've removed the Rokets from Island 4's Icefall Cave, go to the front and use Cut to open the door. Fall through the hole inside, then take the hole to the north, then the one to the west, then to the east, and then to the south. At the bottom is the Sapphire, but a Scientist will snatch it.
