If you mean from FRLG, then migrate the Pokemon to DPPt or HGSS, then use the Poketransfer to bring them to Black/White.
If you mean from HGSS, use the Poketransfer.
You must have beaten Ghetis and found the Poketransfer building before you can transfer any Pokemon.

You can't. You have to get the Pokemon from the Leaf Green/Fire Red onto a DS game before trading.
