What do skulls look like? I mean, are they literaly skulls?
And, is it true you can only find them on legendary?

Yes they do look like skulls (funnily enough). All the skulls other than the one on outskirts which is on any level.
Yes they are literally skulls, and you can find them on any mode.
They just look like skulls, you can only find them on Ledendary, apart from the blind skull, you can get that on any mode.
