04. Death by Chicken (Legend of Zelda: Ocarina of Time)
In LoZ there is a chicken-like beasty called the Cucco - a generally pleasant looking white bird of even temperament and average intelligence - about as smart as your typical Republican - and they have a temper and patience to match!
Now you might be thinking hey, Cucco = food, right? So why not, you know, go attack and kill one, then turn it into a crispy chicken sandwich? Chicken Rage, that's why.
It seems that Cuccos are the gang bangers of Zelda Land. When you attack one in LoZ:OoT after a few hits the Cucco will sound an alarm and the other members of its gang will appear as if by magic and kill you! Seriously, not pretty, not macho, not something you want your friends to see.
republican might match cucco for intelligence but at least liberals are below an eggplant
Drink a thousand bottles of skooma and run to sovengarde and back only to die suddenly once the effects ware off
Dead Space. Being killed by necro dismembered head where it cuts your head off and takes over your body
What's bizarre for me is that this particular death from TWD made it into this list. I finished this game multiple times, and never once have I died making this jump. Never knew I could until I read this list...
Gta4 helicopter glitch when u spawn or buy a helicopter while u r inside it the helicopter bounces around for a few seconds or minute then blows up also it doesn't let u out of the garage and if u enter it

Angry cuccos have been a Zelda staple since A Link to the Past on SNES. If we're going back as far as Ocarina of Time, we may as well go all the way back :P

In the Elder Scrolls III: Morrowind, if you tried equipping Boots of Water-Walking while ankle deep or more in water, you'd instantly be killed.
That game also had the scrolls of Icarian flight, which let you jump ridiculously high and far but wore off before you would land. The result is that if you don't know to use another, you will die if you land on land. I am not sure about landing in water.
Clock Tower series had a few bizarre ways to die. like in the SNES version if you walked up to any mirror and looked into it... some hand would shoot out and randomly choke you. being killed by your own reflection...

Earthworm Jim - death by falling cow. At the beginning of the game, you launch a cow into the air for no particular reason. Much later, that cow falls from the sky and kills you.

In the first, there's that ring beneath the bridge, which you have to jump to, for instance.
Dust 514. As a rookie, call for a vehicle drop, and you see a new marker, you get closer to check it out. BAM! Vehicle drops right on top of the marker, on top of you.
what about the one in silent hill one or two you beat the game badly and end up dead in your car not a mark on you
