To be simple, just die alot in one mission. I'm not sure how many times sorry.

To get the purple heart achievement die at a checkpoint 20 times. It must be the same checkpoint. This doesn't always work at the beginning of a level, so try about half way through. And of course this is easiest on veteran cause you die easily.

For some unknown reason you have to kill your self to get this achievement. About 10 times in 1 checkpoint or something

I don't think killing youself counts
Just run at an enemy trench and don't shoot
