If you're on your first trip, talk to Bill and Celio at the Island One Pok�mon Centre once you finish up what you're supposed to do, and they'll ferry you back. If this is a trip after that, simply go to a port and book passage for Vermilion City. If you are in Kanto, go to Vermilion's port and book passage for Island One, Two, or Three.

It's easy to get off. Go to the south part of the island and keep going until you enter a room with a ship that will take you back to Kanto.
