A) equip up-grade to your Porygon and trade it. It evolves into Porygon2
B) if you're talking about the sevii islands, defeat Blaine on Cinnabar Island (to visit islands 1 to 3) and beat the elite 4 (to visit the other islands). But if you're talking about Johto, Hoenn and Sinnoh then sorry you can't go to this regions in this game alone. You'll have to buy the other games to explore them
