How do you knock down the ladders

On top of the wall get down with an anchor then you just press the circle button. Simple and easy.

Moria: Kill over 2,000 enemies with one character
Fangorn: Collect all the artifacts
Helm's Deep: Beat the game with anyone
Weathertop: Beat the game with all 6 characters

Where do you get the artifact that look kind of like a head and the one that looks kind of like an arm?

You get the head from defeating the witch king sometimes. It doesn't happen often so you may need to keep playing till you do.
