To get the Insane Wang achievement/trophy you must complete the game on the No Pain No Gain difficulty which is the toughest difficulty the game has to offer.

The best way to attempt unlocking the Insane Wang achievement/trophy is to play on New Game+ which will make things easier. What makes the No Pain No Gain difficulty tough is if you die, every single enemy that you have damaged will regenerate all their health. It is because of this it is crucial that you avoid dying. You will however get the most money and XP on this difficulty.
