Do you have to complete the level by your self without dieing to get the costume?

Are you talking about a specific level or in general? In any case you get bonuses from every level for collecting all items and not dieing, and the bonuses do include costumes yes. You don't have to complete it by yourself you can go through the level with other people too.

no you just hit the bubble and get the costume and it is yours!!!
