Complete the following tasks in Single or Multiplayer to receive the corresponding level 5 perk.

Level 5 - Field Medic Perk:

Heal 25,000 HP on your teammates.

Level 5 - Berserker Perk:

Deal 3,500,000 damage with melee weapons.

Level 5 - Firebug Perk:

Deal 3,500,000 damage with the Flamethrower.

Level 5 - Support Specialist Perk:

Deal 3.5m damage with shotguns and weld 250,000 door points.

Level 5 - Sharpshooter Perk:

Get 5,500 headshot kills with the crossbow, handcannon or rifle.

Level 5 - Commando Perk:

Kill 4,000 stalkers with the Bullpup and deal 3,500,000 damage with Bullpup.
