These are unlocked by getting the indicated medals in ALL 25 maps in each land. There are 3 Ex maps per land.

Unlock Map Ex1:

Get ALL Bronze or better in a land.

Unlock Map Ex2:

Get ALL Silver or better in a land.

Unlock Map Ex3:

Get ALL Gold in a land.
