You get the star in the clock for Luigi in a Cage by unlocking Luigi in Course 5- Big Boo's Haunt. You also need to make the red ! Blocks solid. In Tick Tock Clock (the clock), you have to hit a red ! Block so that Luigi will turn invisible and then go into the cave and retrieve the star.
