It is possible to have a child in this game, all though, you have to have a upgraded house, a wife and be married for at least one year. Then Bam! Babie!
Yes, it is possible to have a baby. But, It doesn't grow up. It will always be a baby and you wife will always carry it around her. You will have your baby after 150 days after you get married. Hope this helps!
