How do you get to have a second file star next to your file instead of one even thow you've already beaten the game on the new super mario game for a nintendo ds?

For the second star you have to unlock all areas. Which includes all the stages. Theres also a third star which you get by collecting all the Star Coins. After this I think you need to go buy all of the touch screen backgrounds from the blue Toad's house. Hope this helps.
