At the title screen where it says "Press Start," wnter the following combo: C-Left, C-Down, C-Right, C-Up, C-Left, C-Right, C-Down.

Complete all 3 Bonus stages

To select a different level you need to enter Up, C-Down, Left, C-Right, Down, C-Up, Right, C-Left, L, R, Z, during the intro sequence. You will now be able to select which level you want to play.
