Octopus chart

I need to know where the octopus chart is. I've been looking for 5 weeks now and I have my whole chart filled and I still can't find it.

To get the Octo Chart, you'll need Treasure Chart #26, which is located at Six-Eye Reef. Destroy all the ships and cannons to obtain it.

The Octo Chart will be at Northern Triangle Island.

You have to defeat the nine-eyed octo which is located in one of the southernmost isles east of outset
