Ah, gee, that quest is really obnoxious the first time through. It'll be tough for me to explain, but if you're still having trouble, just go to the Dragon Age wiki.

Alright, you need four "forms" to get through this. The Mouse form, the Golem form, The Spirit form, and The Burning Man form. Each does something different, the Mouse allows you to go through small passages and sneak, the Golem form allows you to break down Massive Doors, Burning Man can walk through fire without dying, and Spirit can go through spirit doors. You need to find the boss in the five originally unlocked areas and defeat them, then go to the three outside nightmares to save your party members, and finally go to the center island to finish the Sloth Demon.

I can't offer a whole lot of help, as it's very complex, but once you have all the forms (Which are given by various dreamers you can find in certain areas) it's fairly easy. Also, keep and eye out for the "Essance Of [Blank]" There are quite a few in the fade, and each one will permenantly increase one of your stats, even after you exit the Fade.
