You just have to do the missions and the pilot trainer is on the third island in a desert place.

You can unlock the other islands (cities) by putting in the lock wanted level cheat and using your jetpack fly to those cities and no tail

You have to complete missions to unlock other islands and the pilot school is in Las Venturas in the desert at the abbandined airport.

You unlock the other islands by completing missions. The Green Sabre unlocks the second island, and Yay Ka Boom Boom unlocks the third. As for flying, you really have to figure out the majority of that on your own. If you wait until the flying school near the middle of the game, you'll never be able to complete it. The flying school is hard.
