You have to lie down(if you know what I mean-press o continuously) and crawl under the tank.

Prone is the only way to move past the Tank. Press and hold O or whatever button you have assigned for crouch/prone and crawl underneath.

You must go prone and crawl underneath. In case you didn't know, you go prone by holding circle (on the default control scheme. On Tactical, it's R3. Other than that, I do not know.). Good Luck!

When the tank falls you just have to huury and prone under the tank and sneak over it.
