The forgotten on can be found by going into the room with all the platforms for the different levels...and by turning around before you go up there go through the door and down the what seems to be never-ending staircase theres also a save-point down there and a door that lead to the torture chamber that the forgotten one resides in...

Go to the Pagoda of the Misty Moon and go to the room with the platforms. Don't jump on the ones thst are lit up. Eventually, you will come across a door. Go into it and you find the Unlock Jewel. Then go tto the warp place where all the entrances to the other levels are and go left and through the door. Go down the stairs and grab the Super Potion and save. Go to the door and it will ask you if you want to use the Unlock Jewel. Say yes. The door is now open and you should go down the stairs. Here is a boss door, the Forgotten One is inside. Be careful, he's a toughie and there are 3 forms kind of.
P.s.-The Ice Whip is the most effective and after you kill him, you get the Black Orb.
