It's pretty easy. After your familiar is level 50 he "powers up" and becomes a really powerful equipable sword. An easy way to level him us I sto keep killing the possesed sword in the reversed chapel. Once he hits LVL 50 he also becomes a lot "smarter" and actually damages enemies by flying through them.
*Note* If you equip your sword familiar he won't be flying in the air beside you, he also won't gain experience. I'm not sure if he becomes a stronger equipable sword as he levels up though. You'll have to figure that out yourself.

And another note, I just found this out last night. When you hit level 90 the sword goes from being a reflective looking green, to a bright glowing white-blue, and becomes "smarter" as well. And the attack power of the equipable sword familiar dose become more powerful as the familiar levels up, but he dose not gain experience while your using him as a sword, only when he is equiped as your familiar. I don't know if the familiars appearance changes again when he reaches level 99, but i'll probably find out soon.
