How do I get past that plank that is in the catacombs that has a level up and a heart underneath?

It sounds more like your talking about the Underground Cavern.
If you keep moving left past that bridge you'll come up to a water fall.
If you have the jump pendant and do it properly, you should be able to jump across to the other side where you can press a button on a bridge similar to the one you are talking about.
It makes a skeleton appear, it will throw a barrel at you , and it will break the bridge under your feet and allow you to get the herald sheild underneath you.
Now that thats done theres a skeleton ape near the bridge with the life max and heart max up.
If you lure the ape out he'll throw a barrel at you and hit the bridge and allow you to go down to the Forgotten Mines, which leads to the Catacombs.
So I just killed two of your questions in one shot, make sure you find the Demon Card, you need it to open a passage to the catacombs.
