Do you have sims1 or 2? Because I don't think they can have kids in the 1st one. You can create a child but to give birth no can do.
If you do have sims2..put both sims relaxing on the bed, click on one for the options of cuddle kiss, woohoo try for baby.

Make sure that your sims have different genders. Also, make sure you are clicking on make baby option instead of just woohooing. If you just woohoo, your chances of getting a kid are slim.
