You need to beat the game again on nightmare mode to get the cow level.

Well you get the nightmare wirts leg and a tome of town portal and go to normal and do it in normal difficulty

You need A tome of townportal and the wirts leg ( you also have to Be in normal difficulty) and in Rogues camp in Act I Go get some friends To beat the cow king because he is Strong.
