On survival you craft it on the Crafting table, on creative you select it from about halfway down the inventory list. You then tap the top of the block that you want the door to be on. To open and shut the door just tap it once.

There is a carpet door that Mobs can't get in, you need signs and carpet, first you put the signs over the blocks closest to the entrance, the put carpet over it, and mobs won't come in!

Oh you have to dig down 4 blocks to put the signs there.
