It's quite simple. First, you need some bait. Go to any island that has the boat floating around it and go into the boat. There you can buy some bait. Then, any island you want to map out you go to that island, then sail around it until you find a fish jumping out of the water. Sail next to the fish and give him some bait. He will give you a map of the island and some important info for the rest of the game. No, there isn't a fish that give you your entire sea chart.
Buy bait from Beadle. Sail around an island until you see a fish jumping around in the distance. Sail up ti him and he will swim underwater to your boat and swim around it. Toss bait in. He will eat it and add the island on your chart. He will also give you some great information about the island too.

Buy some Bait from a Beedle Ship that sails around certain Islands and use that Bait near the jumping Fish at each Island to update your Sea Chart...
Get some bait from Beedle and then go anywhere. When a fish's shadow starts circling your boat give it some bait.
