When putting in a bonus code, do I have to put in ''bonus code'' or just the number? And is the weapons and armor you get from them actually what they say they are? Or a different weapon? Like would aziraals sword of fire be VALERMOS?

Yes, you have to put in "bonus code" to do it. I think you can do it both ways though.
