Once I have made walking stick in hospital and spoken to attendant in hospital what do id o next as I'm stuck can't get any further (thanks)

you need to go to the coblers outside the docs and talk to isaac a jewish guy who works in there about some harnesses

Actually, the Cobbler's Shop is empty when you enter it. You need to examine the harness on the table for the Jewish guy (Isaac) to appear. Even after you talk to him, you'll only be able to get the harnesses off him after you re-visit Whitechapel later on.
Oh, and the shop is southeast from the Clinic's gate.
