I have completed all of the hitman assignments in SR2 but the achievement did not unlock. Just prior to completing them I entered 911 on the cellphone just to see if an ambulance would arrive and it did. Is dialing 911 considered a cheat or is there a glitch in the game?

911 is a cheat. You used cheats, that's why you didn't get the achievement.
