On the mission in act 2 when you meet joaquin carbonell in HQ when he offers the job to kill the 2 rival leaders at the mine...When I accept the job the guard closes the door and wont let me out. Says "not voluntarily this time" or "i would reconsider" and then he doesnt let you out...what is going on here???

Probably just something wrong with your game but when you get to the two leaders kill both of them . Note that the second leader will shoot (but not kill) you or instead he will propose something you can accept the proposal ( I did) but it won't help you.
I did the same thing. You stood in the doorway while he was talking and then moved forward to take the folder. Reload and then when you go in the door go all the way to he desk
