A : The percentage for character D.M.W.s, excluding Genesis, increases with each UNIQUE "memory event" that plays during Modulating Phase sequences involving a specific character. Zack acquires most of the memories as you progress, while others are acquired by speaking to that character at certain point in the game. For summon D.M.W.s, each of the summons must be "power surged" at least once to gain 100%.
