get a cop car, and drive it inside of a hospital then kill all the nurses.

The only way I know of is by completing the game 100%. I don't think there are any other ways

The only way to do this is to make your game completion progress to 100%.

Yes there is but you can only get it by getting one hundred percent game completion and if you have a certain copy of Grand Theft Auto IV than you can also get the fingerless gloves from the cover of the game
