Well first you need a DS with a game slot at the bottom. The first DS and the DS Lite had this. That was taken away when the DSi and later models came out. Now you need to beat your DS Pokemon game and have the National Dex. Now on Route 221 is a building called the PalPark that you go into to catch your Pokemon that were migrated to your Pearl, Platinum or Diamond game. In the Heart Gold and Soul Silver games this building is in Fuchsia City. Now to migrate Pokemon from your GBA game, you need to have the game in the bottom slot and go to your DS game's menu. On it you should see an option to Migrate Pokemon from your GBA game. Click this and it will let you choose up to 6 Pokemon to send over. You cannot send any Pokemon that have HM moves. Anyway after you've sent your Pokemon over go to the Palpark and talk to the guy at the counter he'll give you Pokeballs and let you into the Palpark where you catch the Pokemon. Make sure that you have a Pokemon that knows surf though. Surfing is the only way to catch water Pokemon.

You need to have seen all the Pokemon in Sinnoh. What you can do then is go to the "Pal Park" where you catch Pokemon migrated from your GBA game.
Boot up your GBA game then go to migrate, you need to do this beforehand.

It can only be done on a DS or DS Lite.
