Well it might not be nice to spoil the ending of the Story mode but at the end of it you do end up disappearing because that is how the story mode plays out for the main character. I believe though it's only for a short time.

Somewhat you do. After completing your main game goals, your character will disappear and your partner will return to town, telling everyone about how much of a hero your character was. After what the game says was a few months, your character re-appears to continue doing whatever the heck you want to do in the game.

Yes, you disappear, but after a short video Dialga brings you back as a reward for saving the world.
