Completing the following tasks during the game will earn you the corresponding titles in the Survival Record.
Black Jack:
Clear Horror Hospital in Single Play Mode on Scream difficulty.
Hot Stuff:
Have all your life drained while on fire.
Scream Shield:
Reach Final Fear in Single Play on Scream difficulty eithout an ally losing their life.
When you have completed Fright Factory, Horror Hospital, and Carnage Crossroad you will be able to view the staff
Credits in the 'Options' menu.
