Complete a Tower with every character
The game has multiple towers in both offline and online modes:

For this trophy, you need to complete one tower with every character, including Shinnok. DLC characters are not required for this. You have to manually track the characters you used so that you won't accidentally use the same character. The War Tower is the shortest tower, composed only with 2 opponents and one Test Your Might level. Alternately, you can focus on the Hourly Tower which has only 5 opponents, and allow you to work on the Tower God trophy as well.
Tower Master (Bronze)
Complete 10 Living Towers

Tower God (Silver)
Complete 50 Living Towers
To access the Living Towers option, you have to access One Player from the Main Menu, then select Towers. Living towers has three different modes:
The quickest Living Tower to complete is the hourly tower, which by average will take around 15-20 minutes to complete. Take note that you'll spend around 12-16 hours in this mode alone to complete this trophy.

Reach level 5 in any faction
You need to earn 2,500 EXP to reach LV5. Refer to the No Loyalty trophy.
Faction Champion (Silver)
Reach level 50 in any faction. You need 128,598 XP to max out a faction level.
Refer to the No Loyalty trophy.
No Loyalty (Gold)
Reach level 50 in all factions
Take note that switching factions will reset all XP back to Zero. There's no in-game indicator that will tag the maxed out factions so you have to track them down manually yourself.
Jump Ship (Bronze)
Become a member of every faction
When you start the game, you'll be asked to join one of the five factions. It's strongly recommended to switch factions for this trophy as early as possible since all progress in your current faction will be lost when you switch factions. You can do this by accessing the Main Menu, select Faction, then select Change Faction. You have switch to all of the available factions to unlock this trophy.
According to Leptonic's Roadmap guide for the game, it's recommended to have the following selected in your Kombat Kard:

In a good match with a second controller and Danger modifier activated you can earn around 575 XP in your faction performing a brutality. The game takes like 3 seconds, plus another 15 of loading screens. Here are the step of how to use this farming method:
With this method, it's possible to max a faction level in less than 90 minutes. It will take roughly 7-8 hours completely max out all 5 factions. You can also check out Obie83's method for this trophy.
Time Out (Bronze)
Win a match by time out
You can do this alone by having a second controller. Start an offline match, set the AI to Very Easy, and ensure that you have more health than your opponent. Let the timer run out and this trophy will unlock
Keep it Secret (Bronze)
Find a secret fight in the Living Towers
To trigger the secret fight, you have to select the Living Tower: Premier Tower (Mortal Kombat 1) and meet the following requirements:
After completing the tasks, Reptile will appear. You don't need to defeat him to unlock the trophy; it will just appear right before the match starts.

To view your Kombat Kard, go to the Main Menu, select Extras, and select Offline Kombat History.
Moving Up (Bronze)
Reach personal level 10 in XP
You need 22,000 XP to reach LV10. Don't fuss about this since you'll unlock this as you progress in the game.
Elder God (Silver)
Reach personal level 65 in XP
You need 1,927,124 XP to reach this level, which will take a LOT of grinding. Refer to the No Loyalty trophy for more information about farming XP.
A New Beginning (Bronze)

The game's story mode is composed of alternating cutscenes, QTE (quick-time-events), and matches. Each chapter, you'll use a different character. You can skip fights using Skip Fight Tokens if you just want to get this over with and get the trophies. Besides, completing the story mode will unlock Shinnok. You'll start with three Skip Fight tokens and you'll find more in the Krypt. You can also set the story mode difficulty to Very Easy so you can just breeze through the opponents.
Inner Strength (Bronze)
Win 1 complete online match
You just need to complete one online match, not necessarily win it.
Return Kustomer (Bronze)
Play 100 complete online matches
Refer to the "Unstoppable" trophy.

Giving Respect (Bronze)
Give Respect points in a King of the hill match
(This requires 3 players to boost!)
In the King of the Hill match, spectators are asked to give Respect Points to the King. This is the only way to earn Respect points and there's no way for two players only to earn this. To unlock this trophy, you can just join any random King of the Hill match and give at least one point to the winner.
Respected Fighter (Bronze)
Earn 1,000 Respect points
In the King of the Hill match, spectators are asked to give Respect Points to the King. This is the only way to earn Respect points and there's no way for two players only to earn this. The idea is to have the third player remain as spectator and give the winner of the match respect points. 1,000 points is a long way to go and can be a bit faster with more spectators/ players giving respect points. However, setting up a boosting session for this trophy can be tedious and boring.

Terrifying Encounter (Bronze)
Confront a beast within the Krypt
The Krypt is a separate area where you can spend Koins to unlock stuff. It's explored in first person mode. When you're outside you'll occasionally be attacked by wolves or spiders if you're exploring the caves. You can kill these beasts by pressing the correct button displayed in the screen and earn extra koins. For this trophy, it will unlock during your first encounter, without even pressing any button.

Ranked 1v1 matches can be tedious and since you have to fight against random opponents, having a boosting partner is not much of a help here. However, there is a pretty neat trick by player Hawks1523 that involves saving your win progress via cloud storage:
Step 1. Go to online -> Versus -> Ranked match
Step 2a. If you WIN, go back to main online menu. Press the PS button go to [Settings] -> [Application Saved Data Management] -> [Saved Data in System Storage] -> [Upload to Online storage] OR [Copy to USB Storage] -> [Mortal Kombat X] -> PLAYER SETTINGS -> Upload.
Step 2b. If you LOSE, go back to main online menu. Press the PS button go to [Settings] -> [Application Saved Data Management] -> [Saved Data in Online Storage] -> [Download to System Storage] -> [Mortal Kombat X] -> PLAYER SETTINGS -> [Download]. Yes to overwrite

You have to complete all four parts of the tutorial: Basic, Kombos, Special Moves, and Advanced. You can play through the entire tutorial or skip them, which will get you the trophy all the same. The image below is provided by Leptonic in PlaystationTrophies.Org.
Knockout (Bronze)
Perform a 10 hit combo with every character
This requires multiple retries, and a LOT of patience to unlock. Shinnok is required for this trophy as well. It is strongly recommended to attempt this in Practice Mode. Some characters have X-Ray moves (can be used infinitely in Practice Mode) that can deal up to 10 hits, which will count towards the trophy.
FINISH HIM (Bronze)
Perform 1 Fatality in a match
Fatalities is an integral part of the game so you'll unlock this easily as you play the game. Refer to the Bloody Good Time trophy for more information.
Bloody Good Time (Bronze)
Perform a Fatality in a match with every character
Every character has two fatalities. One is default while the other can be bought from the Krypt. Fortunately, you only need to do one Fatality for every character, it doesn't matter if you'll use the default one or the alternative. Shinnok is also required so completing the story mode is necessary. Furthermore, you need to perform these fatalities in real matches, not in training mode. Using the Easy Fatality ticket to perform a complicated/hard fatality will still count towards the trophy. Refer to this link for more details.

Master (Bronze)
Win a single complete match with every character variation
This trophy can be completed alongside the "Well Rounded" trophy. Each character has three different variations. In order for your progress to be counted, you have to win a match for every character and every variation (around 72 matches). You need to use Shinnok for this trophy as well. DLC characters are not included.
Playing with a second controller is recommended. Furthermore, you can change the match settings to end in one round or within the shortest time possible. If you don't have a second controller to play against a dummy player, you can have a match against AI players and set the battle to Very Easy.
Only a Real Master (Bronze)
Beat an opponent while they still have 90% health and you have 10% or less health remaining
This can be easily done with two controllers. Control the second player and have him/her beat your character until you have less than 10% health remaining. Since the health bar doesn't show the percentage, you have to estimate. When your character's HP is low, use weak, basic punches and leave as little HP as possible. Once done, control your main character and beat your opponent.
Well Rounded (Bronze)
Play every character variation
Each character has three different variations. In order for your progress to be counted, you have to win a match for every character and every variation (around 72 matches). This can be done with the "Master" trophy. Refer to that trophy for more information.
It's a Gusher (Bronze)
Spill 1,000 pints of blood
Don't worry about this trophy as it will unlock automatically as you progress in the game.
Blanche Advantage (Bronze)
Hit someone with the old lady Level Interaction
In the Old Marketplace stage, find the Old Lady in the right side of the stage and press R1 to interact with her and throw her to an opponent. This can be done in Practice Mode or with a dummy 2nd player.
Hara Kiri (Bronze)
Kill yourself with Kotal Kahn for 1 round and win the match
Use Kotal Kahn (any variation) then enter a match with the default 2 rounds. (This trophy won't unlock in one-round matches) Basically, you need to trigger the special move called "Blood Offering" (Forward, Down, Back, Circle. This move damages Kotal Kahn in exchange for a temporary power boost. You need to use this move when your HP is low enough that the move will kill you instead. Now win the match to unlock this trophy.
Back It Up (Bronze)
Equip a new Background image
To view your Kombat Kard, go to the Main Menu, select Extras, and select Offline Kombat History. Press Triangle to edit your Kombat Kard. Select another image as your background image then press Circle twice to save the changes.
Real Icon (Bronze)
Equip a new Icon
To view your Kombat Kard, go to the Main Menu, select Extras, and select Offline Kombat History. Press Triangle to edit your Kombat Kard. Select another icon picture as your Icon then press Circle twice to save the changes.
So Bored (Bronze)
Equip a new Border
To view your Kombat Kard, go to the Main Menu, select Extras, and select Offline Kombat History. Press Triangle to edit your Kombat Kard. Select another picture as your Border image then press Circle twice to save the changes.
Jumping Bean (Bronze)
Jump 30 times in 1 match
This can be done in any mode. Just keep holding the UP dpad or tilt your analog stick up to make your character jump continuously.
Trolling (Bronze)
Duck 30 times during Fatality sequence
In the last match where you successfully completely depleted an opponent's HP, the Finish Him/Her message will appear in the screen. Instead of inputting the commands for a Fatality, press /tilt down as fast as you ducked 30 times before the Fatality timeout expires.

Equip a Background, Icon and Border set
Refer to the trophies Real Icon, So Bored, and Back It Up.
Luck be a Lady (Bronze)
Play 7 complete Test Your Luck matches
This game mode will place you in a match with random modifiers. From the Main Menu, select Single Player, then Test Your Luck. Select your character and set it to Very Easy. Change the gameplay options in Extras to one round per match, with a 30 second time limit to make things faster and easier. Choose a Modifier available then select any map. After winning the match, you can select the option to rematch with the same modifiers. On your seventh match, the trophy will appear.
The Kollector (Bronze)
Unlock 50 Kustom Kombat Modifiers
There are 150 different modifiers in the game and most of them are locked from the start. There are 15 unlocked modifiers from the start that doesn't count so you have to unlock 50 of the locked modifiers for this trophy to pop up. There are two ways to unlock the modifiers: buying them from the Krypt or earning them from Living Towers.

Create a sun ray with Kotal Kahn and perform a flip stance 5 times while in the ray
You can do this while in Practice Mode, in any stage. Using Kotal Kahn (any variant), use the God Ray move (Down, Forward, Circle) then stand in the ray of light. While standing there, keep pressing L2 to change his stance until the trophy is unlocked.
Almighty (Bronze)
Complete a Test Your Might Tower
In this mode, you have to destroy objects in the tower by smashing the buttons. Button-mashing is essential in this mode since the speed needed to keep up increases as you rise up in the tower. The objects you have to destroy are the following:
Not Dead Yet (Bronze)
See all Test Your Might deaths
There are 13 death animations that you can view if you fail the challenges in the Test Your Might tower. The death animations that will be displayed are completely random and it may take roughly 20 mins for you to completely view all of them. You can deliberately and quickly fail a challenge by pressing the L2 or R2 buttons.
Stay Back (Bronze)
Play an Invasion Boss fight

DIE WILL YOU (Silver)
Deal 1,000 total hits during Invasion Boss fights
Invasion boss fights are only active for 24 hours and will pit you against God-Mode AI opponents. Your objective is to land a cumulative of 1000 hits total. Don't expect to win against these opponents; just keep hitting them as much as you can.
INVASION (Bronze)
Complete an Invasion Tower
Select Faction from the main menu, then select Invasion Tower. This mode is not available all the time so if you find it locked, you have to come back for it later. Once available, select an Outworld Tower. In this tower, you have to fight 7 opponents and complete one Test Your Might level. The matches have random modifiers, somethings to your disadvantage. The last two matches are quite hard so you may use Skip Fight Tickets if you're just after this trophy.
Can't Stop This (Bronze)
Win an Invasion 1v1 fight
When playing Hourly Living Tower and Single Fight matches, you'll get a random prompt if you have a 1v1 fight. Accept and defeat the opponent to unlock this trophy. Take note that this prompt will randomly appear if there's an ongoing Invasion. You can view if there's one by selecting Faction from the Main Menu.
