Your post-mission evaluation will depend on the several criteria. Usually, the best way to get a high rank is to finish your mission objectives quickly, don't kill anyone, don't trigger any alarms, not using combat reflexes (slow-motion), no retries (even deliberate reloading of checkpoints) and complete optional tasks (like rescuing other prisoners) if possible.

Rescue

In the “Ground Zeroes” mission, rescued the prisoner to be executed and extracted him via chopper.

Head to the northwest of the refugee camp to find a lone prisoner under the shed. Pick him up and carefully bring him back to the landing zone, just behind your starting point. Call in a chopper and load him there to unlock the trophy/achievement.

Depth

Cleared the “Eliminate the Renegade Threat” Side Op by extracting both targets.
