If you're talking about the revolver magnum, you need to beat the game on any difficulty. Then a mini-game called leech hunters is unlocked. Played the leech hunter game & collect between 30 to 59 leeches then leave the mansion once you have at least 30 leeches to unlock it. Then play the main story again. The magnum will be in the one train room where you find the first save point (the first type writer of the game?)

This is wrong, you need 90-99 leeches to unlock the magnum revolver in leech hunter.Search for more answers for Resident Evil 0 or ask your own question here.
